I suppose we're all middle lane hogs to a degree.  If I'm on a fairly quiet motorway and I can see that I'm gaining on a vehicle in the inside lane reasonably quickly and there is nothing behind me that is going to close the gap on me in the same timeframe, then I'll sit in the middle lane.  Technically, I should probably move to the inside lane and then back again, but it seems a little pointless.  Different circumstances, in my view, when the middle lane hog is not travelling any faster than the inside lane traffic and when being there is impeding other traffic.

Does anyone know the official guidance on undertaking a middle lane hog?

The Highway Code discourages undertaking on motorways with some exceptions (rule 268): "Do not overtake on the left or move to a lane on your left to overtake". Undertaking is permitted in congested conditions when frequent lane changing is not recommended.

That or tailgate, flash lights and wave hand in aghast.

I don't but would be interested to find out. I often undertake middle-laners, at 70 on empty motorways. None of that swerving lanes and dangerous speed stuff, but I see no reason why I should traverse all three lanes to get past them and their irritating misuse of the motorway.
