Middle lane drivers can be infuriating(especially the ones who keep threatening to move over, yet never fully do), but the next person who tries to undertake my van - which has a BLIND spot - I'm going to put you up the fucking embankment.  Driving really close up to people is another stupid idea, invented by people who seemingly would like to die young rather than relax and maybe arrive a little late.  95% of people cannot drive that well(I include myself in this).  Unfortunately 95% of people also think they are great drivers.

thats the most fun, the big fucking blindspot that you can lose a mercedes sprinter van in... i know, i've done it   (i saw him just before i actually pulled out.

my personal favourite is the wankers who sit in the middle lane, doing 70, then speed up when you go to overtake/undertake. then slow down again til you go to overtake, then speed up again etc etc. I almost forced some prick in a Mondeo into the back of a Stobart lorry after he'd done this to me for 10miles...
