The answer to your question....

I have no idea about Ashley Ward (and being a pedant it is not the answer to the question posed anyway), however the second one is correct in that Nathan Blake was the other one who has been relegated 5 times,

1993–94 at Sheffield United,
twice for Bolton Wanderers in 1995–96 and 1997–98,
in the following season for Blackburn Rovers
and in the 2003–04 season while at Wolverhampton Wanderers.

Cannot confirm whether he ever scored 20 goals in a season though.

I was responding to BO's offer of Ashley Ward.... when this question was posed some years ago, the answer was indeed Ward and Blake, but times change.

It was 50% correct.

So the answer now is Herman and Blake.

Robert Earnshaw? Didn't he play for West Brom, Cardiff & Norwich in the Premier League?

Earnshaw, is something like who is the only player to score a hat trick in all Divs + LC FA cup and an international game?

Haven't read through comments or googled.

Both Bolton and Sheff Utd were yoyo clubs in the incarnation of the Prem. Bolton especially. So a long serving player maybe. I'd go for John McGinlay with Bolton & possibly Nathan Blake with Sheffield United.
