Eyes. Anything to do with eyes. Even watching someone putting contact lenses in, and although I'm a big fan of horror films, even I have to look away when eyes are involved. In fact, just typing this is making my eyes feel funny.

For those that are scared of eyes and eye surgery...
I had a squint correction done last year. What they do is cut one of the muscles from your eyeball and sew it back on in the correct position to pull your eye more in line with the other. Once the surgery is complete you had an adjustable suture poking out of the eye so they can make fine adjustments while you are awake.
Quite the experience.

Onset of vertigo possibly. I'm not scared of heights either, but last few years, particularly watching people look off tall buildings in films, causes my stomach to flip over.

I don't get vertigo - done lots of stuff recently where vertigo would have been an issue and not a flutter. Just cliffs. I blame my dad for finding it funny pretending to push me off them as a child. Damaged goods.
