I saw this today:

It might be a bit difficult to see in the picture, but basically some bloke in my office building was fixing a lift when it started to go up. He got trapped and he was pretty much cut in half as the lift rose, what the picture shows is the bottom half of his body dangling from the lift.
I could have avoided seeing it but morbid fascination got the better of me, I'll think twice next time.
