Not quite complete foot in mouth (but pretty close), but I just had an awkward phone call from my deceased step-granddad's daughter, telling me that her mum (his first wife) had died last night.

Someone at the end of the phone telling you about someone who's died whilst you're trying to work out who the fuck they are is definitely not an easy moment. I was close to full on panic before it clicked in my head. I'm not sure "I'm sorry for your loss, but who the fuck are you" would have gone down too well.
