A stop clock would make sense or ensure that the ball is in play for 30mins minimum in each half.
It's normally under an hour over the course of 90 mins.
Could be some long afternoons & evenings if the ball had to be in play for the full 90 plus stopping the clock for stoppages.

Simple solution would be any head injury triggers an automatic substitution. Players will stop faking injury if they know it will end their game immediately.

Or they will hide the injury and play on which is something to avoid.
