I guess to some extent most air accidents are preventable but the cause of this one is a bit bloody fundamental.
What's done is done and I at least those affected can get on with their lives, hopefully having made a (near) full recovery. I'd of thought the pilots licence would be revoked if that is the case. That's one major fuck up, although no doubt the pilot is very much aware of his error and if his injuries were bad, i expect he was unable to keep his flight times up and maybe had to retake his test anyway. I have pilots in the family of both commercial aircraft and private aircraft, i know what proud people pilots are, and also how seriously they take their responsibilities, it's quite surprising that that's the reason.
