Yes to most of it.
The manager in question was Paul Lambert. Think Norwich went up again the following season.
Don’t think Colchester got relegated though.

Colchester finished in eight place. Norwich went up that same season. 

Gordon Greer scored that injury time equaliser against them in our home game.
I was in the Town End that day, the roof nearly blew off when he scored. Grin
