I was at Millwall vs Cardiff last season when a Cardiff fan fell 20ft off the top tier on to the stairs below there. Surprised it hasn't happened before especially somewhere like Millwall. As I've actually thought before that the barrier at the front of the tier is rather low right before it happened.
